چکیده

Objective: To investigate the effects of hypoxia induced by CCHD on the expression and secretion of adipokines. Methods: Subcutaneous adipose tissue and serum were obtained from elective cardiac surgery patients. In CCHD group, n = 17; and in ACHD (Acyanosis congenital heart disease) group, n = 15. Vascular endothelial growth factor (VEGF), inflammation-related adipokines, adiponectin (APN) and Endoplasmic reticulum stress (ER stress) markers were measured in mRNA levels. Hypoxia inducible factor 1α (HIF-1α) and ER stress markers were also measured in protein levels. Results: Immunohistochemical staining showed that protein expression level of VEGF was markedly increased relative to acyanotic groups. Adipose tissue was hypoxic in cyanotic groups. Adipose tissue hypoxia (ATH) led to a substantial increase in inflammation-related adipokines, and a remarkable reduction in adiponectin mRNA levels. Endoplasmic reticulum stress markers were activated by hypoxia. ER stress has been linked to inflammation. These changes were partially reversed by the intervention of normoxia. We exposed normal adipose tissue to CoCl2, a hypoxia mimic, and achieved results similar to the CCHD group. Conclusions: Adipose tissue in CCHD is hypoxic. The change of adiponectin and inflammation-related adipokines are partially caused by ER stress.

THE EFFECT OF CHRONIC HYPOXIA ON THE CENTR AL NERVOUS SYSTEM OF CHILDREN WITH CYANOTIC CONGENITAL HEART DISEASE

Due to advanced treatments in the field of heart disease, cyanotic heart disease patients have a greater chance of survival, and nowadays many of them live until adolescence or middle age. One of the signs of this disease is blood hypoxia, which has acute and chronic effects on the CNS. In this paper the effect of chronic hypoxia on children affected by cyanotic congenital heart disease (CH...

Adipose hypocellularity in cyanotic congenital heart disease.

Height, weight, total body fat, adipocyte size (cellular lipid content), total adipocyte number, and lean body mass were studied in two groups of similar aged children with congenital heart disease. One group was comprised of 19 children cyanotic from infancy. The second group was made up of 16 asymptomatic, acyanotic patients and served as the basis for age and sex adjusted comparisons. Cyanot...

Cor Triatriatum Sinistrum Presenting as Cyanotic Congenital Heart Disease A Rare Case Report

Cor triatriatum is an acyanotic congenital heart disease. We present a rare case of cor triatriatum sinistrum in a 6-month-old female infant who was presented with cyanosis and failure to thrive. The 2D transthoracic echocardiography and the Doppler color flow imaging showed a proximal venous chamber communicating to the distal left atrium  through restrictive opening to the low-pressure, dis...

Influence of hypoxia on nitric oxide synthase activity and gene expression in children with congenital heart disease: a novel pathophysiological adaptive mechanism.

BACKGROUND Chronic hypoxia has been shown to modulate nitric oxide (NO) responses in different cell models, but the relationship between hypoxia and NO synthase (NOS) regulation in humans was not studied. We studied the relationship between endothelial and inducible NOS (eNOS and iNOS) activities and expression and chronic hypoxia in children with cyanotic and acyanotic congenital heart defects...

Nitric oxide metabolism in adults with cyanotic congenital heart disease.

Endothelial-derived nitric oxide (NO) diffuses abluminally to regulate blood flow by activating soluble guanylate cyclase in medial smooth muscle. However, a significant fraction of NO diffuses luminally, where the extremely high reaction rate with red blood cell hemoglobin (Hb) effectively reduces luminal concentration to zero. The erythrocytosis of cyanotic congenital heart disease has potent...
